You will need to complete a Thorndike Partnership new patient registration form and an NHS family doctor services registration form which can be collected from the surgery or downloaded below or by following the below link.

Once completed the forms will need to be handed into the surgery to process the registration.
Everyone is welcome in general practice. You do not need proof of ID, address, immigration status or an NHS number in order to receive care or see a GP.
